This secure portal can be used to submit Client Census Files for COBRA services, after the Client information has been submitted through the Client Information Submission page. Click here for the COBRA Census File Template. 

Please note the following:

• No documents other than COBRA Census files will be processed through this portal.
• COBRA information submissions for a Client will not be considered complete until a completed Census file is received.
• Census files received for Clients whose information has not already been submitted through the Client Information Submission page will not be processed.
